1. Question: What is the Sun Life phone number? Answer: For Extended Health and Dental inquiries about your coverage or claims phone 1-800-361-6212. Your policy number and ID number will be required when you call. You can also register for member access on Sun Life's website at www.sunlife.ca/member or you can go to Benefit Plans, under the Benefits section of this website, for a direct link to Sun Life. 2. Question: When am I eligible for my next eye exam/purchase of glasses? Answer: Vision Care covers you and your dependents for the cost of one eye examination every two benefit years (where a benefit year runs from April 1 to March 31). In addition to that, the Benefit Plan reimburses you for the cost of eyeglasses or contact lenses to a The reimbursement level for the Vision Care Plan is 80%. You can register and go to Sun Life's website at www.sunlife.ca/member and it will provide you with information on your next eligibility date. Please refer to your benefits booklet for more information. 3. Question: Where can I find information regarding my T-4? Answer: The department of Payroll is responsible for any information regarding T-4's. Please contact the Payroll Supervisor at 667-3574. 4. Question: I have a question about my leave accruals, where do I go to get an answer? Answer: Leave records are maintained by the Departmental Human Resource personnel. The only exception is for some auxiliary leave which is maintained by the Employee Compensation Branch. 5. Question: I have a question about my pay, who do I call? Answer: Start by calling your Departmental Human Resource branch, they can answer many of your pay related questions. If your Departmental Human Resource branch can't help you then you can call the Payroll branch in the Department of Finance.
